Using Elexio or Planning Center Forms

The Form module in the page builder supports three form providers: Digital Church (built-in), Elexio, and Planning Center. This means you can embed forms from your church management system directly on your website — no custom code needed.

Choosing a Form Provider

  1. Open the page builder on the page where you want to add a form.
  2. Drag a Form module onto your layout.
  3. In the module settings, find the Form Provider dropdown.
  4. Select your provider:
    • Digital Church — Uses your built-in forms (default)
    • Elexio — Embeds a form from your Elexio account
    • Planning Center — Opens a Planning Center form in a popup modal

The settings below the dropdown will change depending on which provider you select.


Elexio Forms

Elexio forms are embedded directly into your page content. When a visitor loads the page, the form appears inline — no popups or redirects.

How to Add an Elexio Form

  1. Select Elexio as the Form Provider.
  2. Enter your Form ID in the field that appears.
    • This is a long ID that looks like: 4d42b011-c92d-4f4f-a8cc-77e6b2b80b32
  3. Save the module and publish your page.

Finding Your Elexio Form ID

  1. Log into your Elexio account.
  2. Navigate to the form you want to embed.
  3. Look for the embed code or form ID in the form's settings or sharing options.
  4. Copy the form ID (the long string of letters and numbers) and paste it into the Form module.

Planning Center Forms

Planning Center doesn't allow forms to be directly embedded on external websites. Instead, the Form module creates a button that opens your Planning Center form in a popup modal window — the visitor stays on your website the whole time.

How to Add a Planning Center Form

  1. Select Planning Center as the Form Provider.
  2. Enter the Form URL — this is the full link to your Planning Center form.
    • It usually looks like: https://your-church.churchcenter.com/people/forms/123
  3. Optionally, customize the Button Text (defaults to "Complete Form").
  4. Save the module and publish your page.

Finding Your Planning Center Form URL

  1. Log into Planning Center People.
  2. Go to Forms in the left sidebar.
  3. Click on the form you want to use.
  4. Copy the URL from your browser's address bar, or look for a Share or Public URL option.
Planning Center Integration

Make sure the Planning Center integration is enabled on your website. Go to Settings > Integrations in your dashboard to verify. This adds the required Planning Center script to your site.

Always Test Your Changes!

After adding your form, preview the page and test the form to make sure it opens and submits correctly. Try it in an incognito/private window to see what visitors will experience.


Switching Providers

You can change the form provider at any time by editing the Form module and selecting a different option from the Form Provider dropdown. Your previous settings for each provider are preserved — so if you switch from Elexio to Planning Center and back, your Elexio Form ID will still be there.
